Medical Records

For new patients, it is helpful to have all of your child’s previous medical records, including any hospital records.  We can scan these in to our electronic medical record and return the paper copies to you.

For established patients, please let us know if you have been seen in an emergency room, urgent care center or admitted to a hospital so that we can obtain these records to have on file in our office.  It is helpful to let these facilities know that we are your primary care providers and provide them with our fax and phone number.

If you would like to obtain copy of your child’s medical records from our office, in accordance with patient confidentiality laws, we require a written, signed request for medical records release form.  Applicable charges may apply.  We also request 5 business days to prepare your medical records and will contact you when they are available for pick up.
